I know it was after the yoga going 2,11,4 like steps from lagna so it's to be looked from there but i saw in the jamini book that guy was looking at it from the ak being in the first so just wanted to confirm, or id it from both?
It's from the lagna. So if you imagine a line going through the 1st and 7th house axis, you check the number of planets on each side: 2nd and 12th house, 3rd and 11th, 4th and 10th, 5th and 9th, 6th and 8th. Whenever there is an equal number of grahas in both houses within the pair, you get a bondage yoga.

Yes, Rahu and Ketu are included, and considered malefics (the yoga is felt more painfully when there are malefics involved).
