I have noticed that in the case of a transcendental planet, Uranus, Neptune or Pluto - the conjunction ascendant or a strong aspect of it, may even favor a change of name or some kind of performer name or the like.
I don't have such a combination, but in a very massive 12th house, people and even close family members don't understand my nature and life at all, even less familiar or strangers.
@ayan That's a very interesting observation! I have my Ascendant lord Mercury conjunct Pluto. When I got in to the workforce in my 20's, I was kind of forced to stop going by my real name because coworkers couldn't pronounce it/ were uninterested in learning the pronunciation. It got to the point that a new employer asked me on my first day of work: "What's your nickname?"
